HELP!!

I was doing my baby’s nails and I had to pry open his fingers a bit since there was lint and this is between his thumb!! It looks like a blister that popped? :( he has super sharp fast growing nails so I try to keep gloves on him so he stops scratching his face/eyes since he cut open his lip already :( he is 3 weeks and 6 days old. It wasn’t like this a few days ago when I did his nails last? What should I do? He has his 1 month check up on the 19th. Should I schedule something sooner?? :(
