Anyone here who got pregnant even while using the condom and pull out protection at the same time?

My husband and I have used only condoms for 4 years. He doesn’t even pullout with the condom on and we’ve never had a pregnancy scare.
If there are no holes or rips in the condom, they are 90% effective.

My Significant other and I had a condom rip on us it was a bare skin trojan condom. We use lelo hex condoms now that's the only time that happened.
