It means the egg was fertilized, but does not implant. So the test will pick up hcg in your urine, but when the egg doesn't implant, the uterine lining sheds anyway and that's your AF.

It's a super early miscarriage. Most people don't even know they were pregnant because AF can still be on time. 
I had one. I took a pregnancy test one the day AF was due and it was positive, then 5 days later AF started.
