Rash- need advice

So my 9 month old son has this rash that keeps coming back.

Pediatrician says just put diaper rash cream on it.

Dermatologist (went to see for his hemangioma, but mentioned it while I was there) says put some hydrocortisone on it.

I’ve tried both. Many times. Sometimes it goes away, other times it stays for a few days and then goes away. But it always comes back. Been at least a couple months.

I change him regularly. Bathe him daily. Dry him well.

Doesn’t look like typical diaper rash to me, but I’m a first time mom.

Not sure what else to do. I’m not sure if it bothers him. He does reach down there like he’s scratching, but I know boys explore and play developmentally.

Seen anything like this rash? How did you fix it?
