first birth control shot may not have been effective

so i took a birth control shot and i think i could be pregnant he cum inside me and i’m spotting it’s been 18 days and i was on my period for 2 days and i had sex when i ovulated i’ve been having slight change cramps in my uterus and my boobs are swollen my stomach is hard and milk also comes out of my breasts
