Has anyone had an empty sac that got smaller and still had a healthy pregnancy?

3 weeks ago I had pains so I went to the er, and they measured an empty sac at 5w1d and my hcg levels were rising accordingly. Thought I would be about 7 weeks according to lmp but they said I could just be off due to my irregular periods. Besides that all was good. No bleeding.

Went for my “8 week” first appointment yesterday, (which should have measured 8w1d) where I was sent off to a different practice for both my bloodwork and sono. The sonographer wouldn’t let me look at the screen and when I gave her hell, she said “I’m not a dr so I can’t tell you anything” so my response was, “it’s bad, isn’t it?” To which she didn’t reply.

2 hours later I got a call from my dr saying my empty sac was measuring smaller than 5 weeks (this is 3w after my last ultrasound which measured 5w) and that it’ll probably end in miscarriage. That’s all she told me.

My question is, does this really mean miscarriage? Should I still have hope? I don’t have any answers just a possibility that I’ll lose it! Should I still avoid coffee and take prenatal vitamins or can I enjoy my vacation I’m about to board for tomorrow? This is terrible timing. Should I prepare for a MC while I’m out of state? How long does a miscarriage take after you find out the sac has shrunk?
