At what point do you know your bleeding is becoming a miscarriage??

Every time I did the cramps were so bad. The second time I knew I was and got in a hot bath for a while. My baby came out in the sac quickly and painlessly. 
But bleeding doesn't always mean you're miscarrying. My last pregnancy I had early bleeding and then bleeding at 8 & 14 weeks. I had a sub chorionic hematoma. That's what was bleeding. I never had cramps.

I knew when my bleeding turned into heavier period-like bleeding instead of only spotting.

For me the bleeding started off light then got heavier. And the cramps went from barely there to doubling over in pain. But everyone is different. I was 5 weeks 6 days.
