Getting pregnant after a vasectomy

Husband got a vasectomy Nov of 2018 went back for his first sample 2-3 months afyer, all was negative no sperm, but he never went back for his second sample. Your supposed to give two consecutive samples both negative to be considered sterile. Just curious if anyone has gotten pregnant after a vasectomy. TIA
