This has been on my sons scrotum for about a week now.. any idea what it is?

Looks like it's just irritated from rubbing on his diaper. My son gets that too. Try putting some diaper cream on it.

He has a diaper rash, his bum is red also. Get a good thick diaper cream and it should clear right up.
