Squirming, kicking and grunting..

Should I be pulling my son from his bassinet as soon as he starts squirming and grunting for a night feeding? I usually dont pull him until he actually cries because I cant tell if he is actually awake. I swear he will grunt and kick for 30 minutes or so! Is he in pain? Am I bad for waiting?
