Incompetent cervix or just weak organs?

So since I was 14 weeks pregnant to now, I'm 23 weeks, I've had leakage to the point I thought was fluid but was checked and I'm great there. But anytime I laugh, cough, sneeze, get in and out of a car or anytime I open my legs period I pee on myself. Either a tinkle or like a whole bladder full. And discharge is crazy too, sometimes really wet discharge, or really creamy discharge. I want to ask my Dr at my next appointment if he thinks I have a problem. This is my third child but my 5th pregnancy. Each miscarriage happened between 6-8 weeks. I do legals everyday and to the point I actually hurt down there. I can't and don't even want to have sex becahse it actually hurts. The inside of my vagina actually feel bruised and "damaged." Has anybody been through this before or know something. I don't want to wear those bulky leakage pads but I'm about to have to.
