Most important questions to ask at first infertility appointment?

Hi ladies, I’m about to embark on the journey of infertility treatment and am looking for a little advice. I’m 38, my husband is 35, we’ve been TTC for 6 cycles with well-timed intercourse and OPKs and failed, and we’re scheduling our first round of appointments with our doctors for initial semen and blood work. I have several months’ worth of bbt charts to show my OB/GYN, plus photos of all the supplements I’m taking.

Other than showing the doctors the above information and our medical histories and asking about the next steps, what are the most important questions my husband and I can ask our doctors? What do you wish you’d asked your doctor at your first appointment but didn’t? Are there any specific tests I should request? Anything other information I should offer?
