Positive OPK + dominant follicle but no ovulation?

Question

So I had a positive opk on the 22nd. That day, I went to the doc to get an ultrasound to confirm whether or not I actually had a dominant follicle because I have PCOS. Turns out I had a 19mm follicle on my left ovary. My doc figured I would likely ovulate the following day, but based on my chart it doesn’t look like I did ovulate. Is it possible to have a dominant follicle with an LH surge and STILL not ovulate?

Yes I have a regular period and get LH surge and get a perfectly normal follicle growing but despite it all my body doesn’t ovulate. The follicle just keeps growing instead of rupturing and then it turns into a cyst. They found this out during my investigation cycle at the fertility clinic. I most likely never ovulate, even tho all my OPKs are positive. My treatment is then monitoring me and inducing ovulation when egg is proper size with 10,000 injection of Pregnyl then ensuring the follicle ruptured with ultrasound and I did timed intercourse. Then putting me on progesterone gel vaginally. I got pregnant on the second cycle but it ended with miscarriage due to blighted ovum. I am now currently on my third cycle!

Seems unlikely to not ovulate based on what you said but I guess it’s possible. Maybe your temperature was inaccurate. Will you have a cd21 progesterone test to confirm ovulation?
