What does this mean?
I’m from Utah and Utah is not a mandated state for covering ivf. my husband is born without a vas deferens so basically his Sperm cannot swim out of the body. This is noted in our insurance.. so What does it mean by coverage of treating the cause of infertility? Would that mean they could cover ivf? Or sperm extraction? The wording is confusing
