I would definitely do birthday stuff after the shower. 
The one exception would be if the birthday person had something scheduled before the baby shower was announced or if they had some sort of commitment set on their behalf by someone else. Like mom or sister or someone had set up something special for the birthday girl. 
Personally, I wouldn't schedule a baby shower on my best friend's birthday. If I did and they said they couldn't make it because they had something else planned, I probably wouldn't care. They're my best friend. I saw my best friend more than anyone other than my husband during my pregnancy. One day of them not being there is nothing.
