Straight out of the fridge to warm. It varies. We’ve never used a bottle warmer or anything....closest we’ve gotten is running the bottle under hot tap water.

When our baby was a newborn I would warm it under warm to hot water until it didnt feel cold anymore, but I use glass bottles. I wouldn't do it with plastic like that since when you heat up plastic it leeches into food. 
As she got older she was fine with cold milk. She's 11 months now.

Never feed baby hot milk it could seriously hurt them. I only warm my baby’s milk because I think cold hurts her wittle tummy.
