My job doesn’t offer maternity leave so I collected disability during my time off. I live in NJ so I got 6 weeks disability then continued with 6 weeks bonding time, I have been using my vacation days so I have not gone back to work yet. I received a paper in the mail saying I need to fill out a form that says when I returned to work and when I recovered from disability. HR said to put the day my vacation days started for when I returned to work but what date am I supposed to put that I recovered? When the first 6 weeks of disability ended, when the 6 weeks of bonding time ended or when my vacation days started? (There was a week where I got no pay between bonding time and vacation time, my manager never put it for my vacation days those 5 days), sorry it’s so long! TIA!!
