Possible early miscarriage? What do I do

I am 4 weeks 6 days pregnant. Today I’ve had some bleeding. Bright red blood , enough to make a big wet spot in my underwear and for me to put a pad on. I have very very mild cramping but I’ve had this on and off for the last few days. didn’t really think anything of it as I’ve been fighting a bad cold and my whole body basically hurts anyway. Am I having a miscarriage? It’s definitely more than spotting. Do I need to go to a doctor? Do I just let it run its course? I had a first prenatal appointment scheduled for next Wednesday. Do I just wait until then if things don’t get worse? Not really sure what I should be doing right now or if I should be concerned.
