ok i want to have this conversation with this guy i’ve been dating for almost 3 months i want to know where is the relationship going or if its going anywhere at all.. 
 
my question is how should i approach this and what exactly should i ask ?
 
also what are somethings i should look out for in his response that may indicate he not that into me or ready to commit to me. ?

Answer

Make your expectations clear and set appropriate boundaries. 
“Hey—just so you know, I feel like this is going really well and i would love to find out where this goes. I’d like to be exclusive and see only you....”
Then
Option A) “if you’re not there yet, that’s cool. We can keep it casual, but I’ll be either a1) looking for someone who wants to be exclusive or a2) not be dating anyone else.”
*do this until he changes his mind OR you need to break it off
Option B) “if you’re not there yet, that’s cool. But I think we need a cooling off period so that I can regroup.”
Option C) “if you want to try being exclusive, let’s plan on so many dates a week or blah blah blah” and list out what you’re hoping to give and receive in this relationship moving forward. 
Just don’t be offended if he doesn’t feel on the same page as you. If you really like him, maybe slowing down and being patient to see if things change is worth it. 
But if he isn’t there yet and you really need him to be in order to be happy, cut it off. Know that there’s nothing wrong with you, he just isn’t on the same page and that’s ok.

Answer

Be direct and ask him , 3 months is long 
If he says he needs more time to be exclusive you better move on ,you just a option until someone better comes
