Medical advice please

About a year and a half ago, I hit my foot on a stage that resulted in the top area. It’s right on the bone and was a lump until recently. About a year after the first incident, a lump developed under it (the area on the bottom). It has caused a lot of pain, and hurts when it gets hit. I do not have health insurance so a doctor visit would preferably be a last resort. Any ideas or similar situations?
