Teething? Baby screaming bloody murder?

My seven-month-old little boy has went from being a super happy baby to screaming bloody murder throughout the day and night. He doesn’t have any teeth breaking through the skin but I feel like his gums look a little swollen and he is super drooly. This has been going on about a week now. I took him to the pediatrician and they have ruled out an ear infection because he has been grabbing his ears having a hard time sleeping but has no fever. The doctor said usually teething will only last for a few days of them being super uncomfortable and usually they will have a tooth breaking through the skin. If he is still having issues they want to see him back at the end of the week. Has anyone else had their baby have prolonged very uncomfortable teething? Or could this be behavioral? A leap?
