How do I tell my boyfriend I tested positive for chlamydia?!

Back in January I tested positive for chlamydia from a past relationship I got the pill and was told to come back for a check up in April, unfortunately I never got my checkup to see if I was clear because of the coronavirus pandemic. After my ex I hadn’t had sex since then.... I am not in a happy relationship and we have started having sex first it was protected and then we had made the choice to not use protection.. 2 weeks ago I have started feeling the same symptoms from last time and I was so horrified I prayed to god that I didn’t have it still I got tested and was told I had a UTI I was so relieved then I waited a week for my swab test results to come back and I got positive again Im so broken because I am so sure I probably gave my boyfriend chlamydia and I’m so sad to make him go through the same damn horrible situation I went through I got the call today and I plan on meeting up with him to tell him I’m just not sure how to tell him and how he’s going to react I had let him know about my past situation so he know that I had gotten it months back but that I wasn’t sure if I was clear so please you guys help me!!! I don’t know how to tell him these weren’t my intentions at all I am so disappointed in myself for putting him through something so terrible I really can’t stop crying ughh I hate myself for thisssss.... I COMPLETELY understand if after this horrible new he doesn’t want to be with me anymore it going to hurt but it’s my fault 😔
