What would you do?

We adopted an older dog in 2018. His teeth were in very bad shape and we took him in for a cleaning and extractions about a year after getting him (we tried brushing and stuff, didn't work). When we took him in the vet said he was at least two years older than the shelter said he was, making his age about 10 years old then.

Fast forward to now, the rest of his teeth, even though we still brush regularly, are shocking. If we take him in again I feel like they might extract the rest of his teeth. However, the one vet we took him too said he wouldn't bother with putting him through that at his age (he's about 12 now) and to just keep brushing and stuff.

His breath is really bad though and his teeth are really green. When we brush, even though we use a soft toothbrush, his gums bleed. We also spray colloidal silver in his mouth.

I'm thinking of rather taking him back to the first vet and asking for their opinion again, but I'm worried they're going to judge me for leaving it and that the stress of going under again will be too much for his age.

He is a big dog so he probably doesn't have much longer to go. As it is he's starting to get incredibly anxious and nervous lately, and his eyes are getting milkier.

What would you do? He doesn't have any issues eating, he gets biscuits everyday (does nothing for tartar control) and he gets a nice meal at night, plus many little treats during the week.
