Restraining order advice

So I found out my child's father what's doing drugs Xanax he's also has a past of drug abuse he is bipolar and addicted and I just found out after being away from a week what a narcissist is in that is him on top of all this I do have a restraining order on him at last until the 22nd to get it extended

I am not sure how this is going to go I would like to extend it for a year I want to ask for supervised visit for him to attend psychiatrist for him to pass drug test I also want to put him on child support? Any other suggestes you have or any advice please I'm listen
