Sore is making BF painful

I have been breastfeeding for four months and I woke up yesterday with an extremely tender nipple. It has a small red spot near the tip with maybe two small white dots near. It is hard to tell since it is so painful to touch. Are there any ideas what it might be and how I can go about helping? Thank you!
