Help Pleasee!

So my son use to take pacifier then at like 10 1/2 month he decided he no longer wants them. Which would be great if he wasn't replacing it with my boob. He literally nurses all night and pinches and scratches my other nipple to fall asleep. He has sleep issues so he still wakes up like we are in the newborn stage. He has an appointment with a sleep specialist. But I really need to know how to get him to stop using me as a pacifier 😩 I have never had any pain breastfeeding everrr until now. My nipples hurt so bad from the pinching and scratching and never getting a damn break.
