When did you switch from infant size nipple hole?

Never.  No need to unless their sucking in air or getting frustrated.

Same as above! I think its only necessary if you see the nipple folding in bc he’s sucking too hard. Never went past the preemie one lol I think it’s kind of a money gimmick in my experience. I figure the mothers nipple never goes up in size so unless your baby is showing they need it you don’t have to go up in size either

When baby took forever to finish her bottle. I think it was around 6 months

I never did either. If you’re breastfeeding you don’t need to is what I was told. Unless you have a really fast let down you might up size to match. But you can just keep it to the smallest nipple hole usually and they will get milk the same speed as when nursing.
