Dry nipples

This may be TMI for some but I've noticed that my nipples are extremely dry and I think the dead/dry skin blocks my ducts. I've been using lotion pretty frequently but it doesn't seem to be helping. Last night I managed to clear them out and started leaking colostrum I believe but it made them sore. Anybody know what I could do to clear them out or something?
