Relationship help!?

So me and my fiancé have been together for 2 years. He never willingly loves up on me (kissing , hugging, cuddling),

I’m always asking him to be intimate with me and he always acts like it’s funny or it’s a game. Even when I try to “touch” him (🍆) , he always pushes my hand away or makes it to where I can’t touch him. He has kidney issues which he’s blamed all of this because of that. I just don’t understand, I’ve been with him for 2 years now and it’s always been this way. I’m always have to beg to get some or beg to even be touched. What do you guys think ? I try to have him open up with me a little more but he never tries to make things better. He never willingly touches (😸) even after I ask him to.
