You get the forms, sometimes they are online, I know for the initial packet at least in my county, they don’t charge but for modifications they charge $25, I would contact by phone somebody there first if you could, or by email, make sure you get the right forms whether you are married or unmarried it’s different forms, and then you mail them in with all the proof and evidence, your birth certificate, the kids birth certificates, the kids medical insurance cards, copies of their socials and yours,  One month of pay stubs, three months bank accounts, your passport, if you have one, your drivers license, your taxes, with the W-2s, and you definitely need to have a very good narrative come and file a motion￼. Some people do it on their own, some people use services, if it’s due to abuse or neglect, you can also contact Womans Space or whatever domestic abuse shelters or community centers they have usually an advocate to help you complete the forms, also counseling is probably recommended as well, not sure if there’s any immigration necessity as well, if you are in need of a victim visa that’s also where they would help you and the prosecutor has to sign off on it, I would definitely sick some counseling, the children may need counseling depending how old they are, and I would go for sole custody and child support.
