Depends on how she sits in it. It should come out right at her shoulder or below it.

First off is she forward facing or rear facing. For rear facing it should be at or JUST below the shoulders. If forward facing it should be at or JUST above the shoulders

It completely depends on where her shoulders are when sitting in the seat and whether she is rear facing or forward facing. At 3 if your primary concern is her safety she should be rear facing until she reaches the maximum rear facing height or weight limit of the seat. Her vertebrae are not fully developed and even 1/4 inch of stretch in a collision could sever her spine and internally decapitate her. When rear facing the shoulder straps should exit the car seat shell AT OR JUST BELOW her shoulders when seated. 
If she has maxed out the car seats rear facing weight or height limits and is forward facing, the straps should exit the car seat shell AT OR JUST ABOVE her shoulders when seated.
