What does first prenatal appt consist of?

So I have a OBGYN appt in 2 weeks, and I’ll be about 8 weeks pregnant. When I made the appt, I asked if they are doing an ultrasound and they said first appt will be a physical and blood work. 
Can anyone tell me what consist of the physical? And why wouldn’t they do a ultrasound? How can I know baby is doing okay?? I thought all places would do an ultrasound first appt??

They don't usually do them that early without reason because it tends to scare more than comfort the mom. You can't see much and you may or may not be able to hear the heartbeat just yet.

I think thats weird i always had a ultrasound with my first appointment and my understanding was for dating purposes. To measure how far along you are.

Mine was like a normal annual with extensive medical history taken, STI screening, and general pregnancy discussion. 
My ultrasound was at my next visit at 10 weeks. 
With my first my first prenatal and ultrasound just lined up together but didn’t get so lucky with this one.

I've had a few with my clinic. They start with dating/viability scan. Urine sample, blood pressure, weight check. Wait for the OB. Breast exam, pap smear (we only do the pap because I'm always due for it), questions/concerns, binder full of info (what to expect at every appointment, pregnancy safe medications, etc...), talk about my due date. She leaves and sends me off down the hall for blood work. They wrap me up, I schedule my 12 week appointment, and I'm on my way.

Many places are moving to only one ultrasound during pregnancy at 20 weeks for the anatomy scan. Most will do 2 total, the first at around 10-12 weeks. Anything up until then is with a Doppler to check the heartbeat.
My first midwife appointment (9 weeks) included a tour of birth center, some paperwork with the nurses, I had about an hour talk with them and my midwife to ask questions and get information. I did happen to have an ultrasound but that was only because my progesterone levels came back low and they wanted to make sure it wasn’t a miscarriage. If your blood work and symptoms don’t show any warning signs, there’s not really a huge medical reason to do an ultrasound that early.

I didn’t have a physical at mine but I did get a Pap smear. She took down my general medical history as well as any medications I was on. Did a urine test. Answered my questions. I got a very quick ultrasound which seemed like it was a last minute decision on her part to try and make up for being 30 minutes late to my appointment. They didn’t have the necessary equipment to do a “good” early ultrasound so I was referred out for that. I had to make a second appointment with her nurse to do the bloodwork and go over my birth plan and all that stuff.
