Is there always a nurse with your ob/gyn while getting an annual? Male or female ob/gyn. First time i didn’t have nurse in the by room and I have a female doctor.

Only when it's a male OB/GYN. Never have with a female. I think it's standard practice with the opposite sex to have a nurse present. Protect both parties.

It depends on the protocols of the practice. Some want a nurse as a witness so they don't run in any legal issues later (some patients lie about the doctors bedside manner and claim to have been a victim of inappropriate conduct).

In my country obgyn works with a nurse-midwife. So both are always in room. Midwife usually wrotes down all the stuff ob tells in records. And so some simpler tests. There is always two persons in room, no matter of doc is male of female. I once even had a male nurse-midwife.

I think it depends on the office. My previous gynecologist office always had the gynecologist and a nurse in the room. They wouldn’t complete cultures without it. I actually hated having a nurse sitting there. One literally just stared at my vagina the whole time once. It was weird the way she stared. Made the whole appointment more awkward than it already is having somebody in your vagina. 
At my new office (switch due to moving) there’s never two. Only the gynecologist.

Ive never had a nurse in the room for an annual. Its only ever been the midwife or OB. The only time ive ever had a nurse during anything like that was my GBS swab for my second pregnancy but my husband was in the room as well so i dont think it was really an integrity thing

Nurse is always in the room when my clothes come off. If my clothes are staying on, then no.

My doctor is a male and he always has a nurse present when doing exams. If there isn’t one in the room at the time, he calls for one. 
If your doctor doesn’t and you don’t feel comfortable with that, say something.
