My boyfriend is not "as attracted" to me any longer 😥

My significant other told me he is not as attracted to me any longer. I found out I have Insulin resistant type pcos and I gained alot of weight from it until doctors could figure out what was wrong . I have been steadily loosing it but it takes time. He is a truck driver so I rarely see him, yesterday I picked him up at the terminal (truck terminal) and we went out. I dressed up feeling confident in myself for a change, and I really wanted to impress him. We haven't been intimate in months since he has been working so much. He wouldn't walk near me while we were out, always ahead of me and told me he was embarrassed to be near me because of how I look and my height with my heels on, I felt horrible about myself. I'm a first responder and this year has destroyed me, my health spiraled out of control, I was broke, I left my abusive family, moved across the country and I've been working non stop. I have been stressed. I don't think I deserve this type of treatment because he has things that he can't control about his physical appearance and I never see him as anything less. What should I do? I feel so broken down, my confidence is gone.🥲 I'm doing my best to loose weight, any advice on weight loss or relationships would be appreciated!
