Looks like a popped blister. Are his shoes rubbing his foot in this spot when he walks? If he keeps wearing the same shoes it will keep irritating it.

It is infected. The red around it indicates it’s infected. Looks like a blister from his shoes. But some antibacterial on it with a bandaid. Might need to put some mole hair in his shoes or let him be barefoot for a while.

I agreee it looks like from the shoes.  Use a different pair of shoes and put a bandaid over it to minimize the rubbing
