Rash?

Hi everyone. Need some advice. My 1yr had never gotten a diaper rash (thank god) until now. Well at least I think it’s a diaper rash. It’s super red around his anus. With bumps on each side and some bumps going up his butt. They look pimpleish. Some have like white in it. Is this something to be concerned? He has his 1yr doctors appointment on next week Tuesday. I’ve been adding diaper rash cream but haven’t seen any improvement. Any advice. I have a picture I can send but not sure if it’s allowed lol
