Chronic Yeast Infections, BV, UTI

I'm just unsure what to do now, I've been crying myself to sleep for the past month trying to solve this.. I've been dealing with chronic BV, Yeast infections and UTIs for about 2 years now.

I've switched gynecologists twice now, because they all keep saying it happens. I get them when I'm not sexually active with my partner and when I am. Sex itself is painful now and we were fine our first 2 years (been together for 4 now). This all started when I forgot a tampon in 2 years ago for 4 months. I'm so thankful I didn't get toxic shock, but now I feel like I'm cursed. I've tried everything.. we've switched our diets, probiotics made it worse, supplements, boric acid.. everything. I do have a iud, kyleena if that helps!! Any suggestions? How do you guys force the gynecologist to run tests? I'm only 23 and I really don't want to deal with this the rest of my life. 😭
