Diaper rash

Hello everyone!!! First time mommy stressing out here! My baby girl has a pretty bad diaper rash. It obviously wasn’t bad in the beginning I put a little cream on it but now it seems to have gotten worse to the point where her skin is broken and bleeding a little. Idk what to do to correct it and I’m freaking out. I know it hurts her every time I wipe. The dr said to just keep putting diaper cream on it and it will help and to layer the stuff on but it’s not working. Does anyone have any suggestions on how I can fix this for my poor baby girl.
