Wondering if I just miscarried

No cramping but was going to the bathroom and heard something plop into the toilet. Took a piece of toilet paper to move it around and looked like brown tissue. Just been spotting brown today. 5 weeks pregnant.

Doctor told me to prepare for a miscarriage since I’m spotting brown and my hcg isn’t rising quick enough.

Is it possible to miscarry without cramps or bright red blood?

I also just had a pelvic exam so idk if she might have loosened something up.
