7w1d Ultrasound

Hi everyone! My husband and I went in for our first transvaginal ultrasound today.

Before my appointment I had quite a bit of pink discharge with clots this morning and lower right sided cramping.

Gestational age is 7w1d and the baby is measuring around 5w5d with no heartbeat yet. Could tell that the doctor seemed a bit concerned after the scan given that this is a high-risk pregnancy (type 1 diabetes, hypertension, and Hashimoto’s thyroiditis). I’m definitely at high risk for miscarriage. Is this normal to not have a heartbeat yet? What does it mean when the baby is measuring too small?
