What does it mean when people say “they’re afraid to go under anesthesia”?

English isn’t my first language and I’ve always been confused.
Can anesthesia make people die and that’s why they’re afraid?
I’ve also seen how some say that they had family members or friends who never came out of it. Do they mean out of whatever surgery in general or specifically because of the anesthesia?

both. anesthesia scares people because sometimes you can die under it. others are just scared to be artificially put to sleep.

It could be both, sometimes people don’t survive surgery, sometimes they don’t come out of the anesthesia or wake up, sometimes complications happen, sometimes medical errors happen, that they have to re-open you up again, because they left something inside of you, like a sponge or some thing else, sometimes people go into cardiac arrest while they are in surgery, sometimes they puncture your lung, sometimes anything is possible bleeding out, a transplant not working, because it rejects it, it could be a number of things. Sometimes there’s so much damage, that they can’t fix things, for instance if you were a smoker for 40 years, you have COPD, you have emphysema, you have internal bleeding, a number of things, you keep getting pneumonia in your lungs keep filling up with fluid after they drain it, eventually things get out, and you are gasping for air and eventually you can’t take it anymore you can’t breathe with machines or without machines.

For me I refuse to be Put to sleep unless I absolutely have to be. I was awake and on "laughing gas" for my wisdom teeth removal
I don't like the idea of someone screwing up and giving me too much and killing me, or undergoing surgery and dying while I'm under 
It's a hard no for me

I’m scared of anesthesia because of the completely vulnerable position it puts you in. Deeply asleep with no control over what happens.
