Vaginal Tear after sex

Every single time I have sex I always get a little tear at the opening end of my vagina. I hate it!! It always makes it so much more painful to have sex! Does this happen to anyone else?? What can you do to fix it?? We always use lube but I still get a little tear. Any help would be great.
