Extreme ADHD, would you medicate your child?

My 5 year old son has a level of ADHD that even his therapist is shocked by. He cannot pay attention to anything for more than a few seconds. His emotions are extreme as well and he goes from zero to seeing red within seconds. Ask him to do something as nicely as possible? You get screamed at. His doctor said this all related to the ADHD and what is called emotional dysregulation. He cannot control his emotions any more than he can control his attention. If you say something like “hey honey can you pick up that block from the floor?” he responds with “NO!! AHHHHHH!! LEAVE ME ALONE! IM NOT DOING IT!!” and he’s like that with everything. We have to walk on eggshells around a 5 year old child. He bounces off the walls, furniture, constantly breaks his own belongings and ours. His kindergarten teacher cannot get him to sit in his chair or stop moving around and he won’t do any of the assignments unless he is being hovered over and he screams/cries about it the whole time. We have the option to medicate but I’m so fearful of it affecting his development or brain or something. I’ve heard nothing good about ADHD medication. But his behavior is out of control and I don’t know what else to do. He hits, screams, cries, about everything. I basically have to leave him alone the entire day or he goes into hour long meltdowns of screaming and breaking things. Would you medicate? (Before anyone tells me to get therapy- he is in therapy, he sees a psychologist and an OT).
