why would a mature follicle fail to release egg?

I guess I have been told that a follicle can grow to 22-24 mm and contain an egg but then fail to "release" it. and I was wondering why this happens. or do mature follicles typically release their egg?TIA

Like Heather said, it's usually a horomone issue that causes failed ovulation. There's a "sweet spot" with follicle and egg development. If a follicle that houses an egg releases too early, it won't establish meoisis properly (think chromosomes here). Conversely, if a follicle that houses an egg grows too large it becomes cystic and therefore no longer houses a viable egg. In very rare cases, even 1 trigger shot won't release the egg and multiple are needed. Though, this is actually user error in most cases (not mixing the trigger correctly for example- super easy to do!). 
I'll just give you my case for example. I have PCOS. Before I had my ovaries drilled my LH was double that of the FSH. When I would take ovulation induction meds, my hormones were so whacky so nothing would grow. My FSH targeted all my follicles instead of choosing a dominant one. This was in part due to that high LH as well; it never climbed to tell my body to release an egg. Post ovarian drilling, my FSH was in overdrive since it was re-learning what it needed to do. As a result, my follicles grew so fast the first few cycles. By the time I'd be getting my CD10 scan for trigger, I'd have a 28+mm follicle. It took awhile for my hormones to regulate and not fight one another for everything to synchronize. I still had to trigger to try and avoid cysts but the eggs weren't viable until like 4 months post surgery and then I started ovulating on my own.
