I'm all for homemade things. Are you very close to the couple? 
Crochet a blanket, gift basket of homemade canned preserves. Think outside the box. Would they enjoy passes to a nearby museum? Or a gift card to their favorite restaurant?

Every wedding we go to we put money in a card and then the person who receives the money or gifts can decide what they really need.

We were together five years before getting married and fortunate to have most everything. However, most of it was not super nice sets and somewhat throw away and starting to wear. We loved everything we got ... A registry is there for a reason because they already stated they wanted those items. Although, our favourite gift was a set of leather luggage tags custom made. We travel 6x or more a year and used it right away for the honeymoon!
