It's when the run their fingers between your cervix and sac. It releases some hormone and it's supposed to help labor start. At least that's my understanding.

Asked my OB about this yesterday. I am 37 w 3 d. She said that the efficacy can vary from person to person. For example, my 
Doctor is pretty petite and has small hands, so it is a bit more challenging for her to effectively separate the membranes. She said one of her former mentors was a larger male doctor and was much more effective at inducing labor with this method. She said she would not attempt it until I was a few cm dilated.

Most Drs won't do it until 38/39 weeks

All my friends actually had this down at 38-39 weeks and they went into labor like one or two days after. Perfectly healthy babies! I asked my midwife and she said she doesn't believe in it...and wouldn't do it until like 41 weeks if she had too....I'm pretty pissed about this.
