Needing some help.

I realize this may it be the best place for this but I posted this same question in different room and no one answered. 
I had a baby in November, afterwards when I was home for a few weeks I started to notice that my concentration has almost vanished. I cannot get anything done.  I'll start something to only completely zone out minutes later. I barely can get any of my studies done for college and barely finish any house work. It's to the point where I'm afraid to drive because I'll zone out sometimes. 
I have full intention to finish things that I starts but can't and then when I realize that I need to finish things, I get overwhelmed because I have a lot of things to finish.
I know having children ( 2yo and 2 month old) can be challenging and mentally draining at times but my 2 yo is on a very set schedule and my 2 month old is sleeping through the night for sometime now. 
I've been spacey for a while now but just chalked it up to be pregnancy brain and being tired.
I have no idea what is wrong with me but I need to get this fixed asap.
I don't have a medical doctor and haven't for a while and am afraid that if I make a new patient appointment for zoning out they won't take me very seriously since they don't know me at all.
could my gynecologist diagnose me and help me? I'm only asking because she's been the only doctor I've seen and knows me fairly well.
