Looks like you just had a lot of wax stuck up in there. You should try getting a wax extraction. 
Also keep in mind that it is healthy to have some earwax and Q tips are damaging to the ear canal.

How can clean my ears like that?

Eeeeek! I've been partly deaf in my left ear for a week now and have been given drops to soften the wax. This just scared the sugar out of me! I hope mine isn't like that😫

Could be an infection so you should go see your doctor
