He has had it forever. His pediatrician said it could be eczema and prescribed a cream that hasn't worked. I've tried changing body wash but it doesn't improve. Help please.

If you are bf, maybe some breast milk. When my son had a similar heat rash or baby acne (not sure which it was) coconut oil helped it disappear in a couple days!

Could be eczema, I can get similar looks spots? Is he scatching? And what cream did your doctor give? A hydrocortisone?
