My baby hasn't ever had a diaper rash so is this what it looks like and if so what's the best thing to put on him.

Stop using regular baby wipes, use Water Wipes brand or just washcloths and water, use Triple Paste with every diaper change and change diaper often. After rash has gone away, use Aquaphor at every diaper change to prevent rashes. Stopping regular baby wipes, even Pampers sensitive wipes (full of chemicals), helped stopped rashes on my LO, I only use washcloths with water.

If you want a cheap remedy, brown some flour in a skillet and rub it on the babies bottom. Gone in 3 days. Worked for all my babies.
